CFA Victoria 2011, crew was suspend pending an investigation. Truck had to be completely stripped and rebuilt. The crew were responding to an Alarm in a building and not a fire.

I'm not sure of the outcome of the investigation. It wouldn't just be the driver. A fire truck is kind of like a ship where the OIC is in charge. The driver is just that the driver. The Officer in the passenger seat is ultimately responsible for the truck, Crew and crews actions.
